Planning Your Rhinoplasty

The ideal candidates for rhinoplasty individuals who are unhappy with the size, shape, appearance, or functionality of their nose, and who have a realistic expectation of the procedure’s success (a rhinoplasty will enhance your natural beauty, but may not transform you into a movie star!)  Individuals should have finished growing (girls should be at least 16-17, boys may need to be a bit older) and should be undertaking the procedure because they want to, not to please parents, family, or friends.

The initial consultation for a rhinoplasty is critical, because this is when you will work together with your surgeon to design your ideal nose.  You should plan on bringing photographs of noses that you find attractive, and you will also be asked to look at many photographs provided by your surgeon.  The goal is not to identify “THAT NOSE”, but to give Dr. Motykie a complete understanding of your ideal of beauty, so he can work with you to design a unique nose that will blend perfectly with your facial features and bring harmony to your profile.  Every individual is unique, and a cookie-cutter approach to rhinoplasty is simply not acceptable.  Dr. Motykie will listen to your aspirations and concerns and work with you to design your unique nose.  He offers computer imaging to help the patient and surgeon make precise adjustments to the surgical plan and visualize the final results.  This may be especially helpful for patients who must travel for the surgery and cannot have an in-person consultation in advance.

Rhinoplasty Procedure and Recovery

Rhinoplasty can generally be completed as an outpatient procedure in the surgeon’s office, although a short hospital stay may be required after especially complex operations.  The procedure can be performed under a local anesthesia, when you will be awake and relaxed, but your face will be numb and unable to sense pain, or under general anesthesia, when you will sleep through the procedure.  You and your surgeon will discuss these options and together decide the correct choice for you.

Unless especially complicated, most rhinoplasties can be completed within an hour or two.  The procedure usually involves small incisions inside the nose and/or at the base of the columella (the strip of tissue separating the nostrils) to avoid visible scarring.  Once the skin of the nose is separated from the bone and cartilage, the structures of the nose are reshaped according to the surgical plan, and then the skin is re-draped over the new framework and sutured in place.  A splint is applied to the nose to help it maintain its new shape, and nasal packs may be placed in the nostrils to stabilize the septum.

Most patients will notice swelling and bruising around the nose, eyes, and cheeks, which may increase for the first several days after surgery but should subside within two weeks.  Any discomfort can be relieved with doctor-prescribed pain-killers, cold compresses, and elevation of the head.  Minor bleeding and stuffiness is common immediately after the procedure, and you should refrain from blowing your nose while the tissues heal.  Most patients are able to return to their usual activities within a week or two after the surgery, but find it takes several weeks to get fully back to speed.  Wearing glasses on the bridge of the nose should be avoided for six or seven weeks after the procedure.

In a limited number of cases, patients may find that small burst blood vessels appear as tiny red spots on the surface of the skin; these are usually minor by may be permanent.  Additionally, in about one of ten cases, a second, minor procedure may be required to correct a small deformity in the new nose.  Such complications are unpredictable and happen to patients of even the most skilled surgeons.
